Diagnosis and Management of TMJ (Part 1): An Integrated Approach to Orofacial Pain

This course will define temporomandibular disorders (TMD) related to diagnosis and conservative interventions. The history of TMD and its medical management will be provided. Major types of TMD as defined by “Diagnostic Criteria for TMD”, or DC/TMD will be examined. Relevant anatomy, physiology, pathophysiology and biomechanics of the TMJ related to enabling a thorough clinical examination will be reviewed. Examination processes, including basic clinical examinations, special tests, imaging and differential diagnoses will be detailed. This will assist clinicians to better recognize and treat the primary variations of TMD. Options for conservative management will be provided with specific attention to manual interventions. The necessity of a multidisciplinary approach to treatment will be presented. This multidisciplinary approach will include the dental and biopsychosocial aspects that frequently accompany TMD.

Course Outline/Topics

  • Name and identify the structures that comprise the TMJ.
  • Describe the normal biomechanics of the articular complex of the TMJ.
  • Utilize the DC/TMD to classify sub-groups of TMD.
  • Differentiate the clinical findings of masticatory muscle dysfunction from those of TMJ joint disorders.
  • Explain the role of TMD in certain headache disorders.
  • Propose a specific treatment plan for each of the main sub-types of TMD.
  • Determine the general postural components and potential dysfunctions within the cervicothoracic system as contributors to TMD.
  • Explain the role of central sensitization in the etiology of some forms of TMD.
  • Explain the need for a multi-disciplinary approach to the management of more chronic forms of TMD.
  • Identify the correct patient/clinician positioning for completion of the TMJ examination and for mobilizations.
  • Demonstrate performance of the basic TMJ evaluation independently and in a timely fashion.
  • Demonstrate performance of both TMJ mobilizations and soft tissue techniques.

Prerequisites

PRE-COURSE ASSIGNMENT: It is recommended that participants review Chapter 4: Temporomandibular Joint in “Orthopedic Physical Assessment, 6th Edition, by David Magee, PhD,BPT.

REQUIRED COURSE MATERIALS: It is recommended that participants bring a copy of “Gray’s Atlas of Human Anatomy”, “Netter’s Atlas of Human Anatomy“ and/or comparable text for reference (although the presenter’s lectures will include slides with illustrations.
