Form Management"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\" id=\"h-purpose\">Purpose<\/h2>\n\n\n\n<p>To ensure consistency, efficiency and accuracy across all Slate forms used for marketing and recruitment, the Division of Marketing&nbsp;&amp;&nbsp;Communications (MarComm), Office of Student Enrollment Communications (OSEC), Graduate School and Center for International Education (CIE) have established a formal structure for form ownership,&nbsp;setup&nbsp;and maintenance.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-slate-nbsp-form-nbsp-management-nbsp\">Slate&nbsp;Form&nbsp;Management&nbsp;<\/h2>\n\n\n\n<p>Responsibility for Slate form management includes the following activities, which ensure forms&nbsp;remain&nbsp;accurate,&nbsp;functional&nbsp;and aligned with institutional needs.&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Initial setup and configuration of forms&nbsp;<\/li>\n\n\n\n<li>Ongoing maintenance and end-of-life archiving&nbsp;<\/li>\n\n\n\n<li>Review of form fields at least annually, with more frequent reviews as needed<\/li>\n\n\n\n<li>Updates to fields with overrides of system prompts&nbsp;<\/li>\n\n\n\n<li>Creation and maintenance of confirmation emails&nbsp;<\/li>\n\n\n\n<li>Creation and maintenance of redirects&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>The following table&nbsp;outlines form&nbsp;management responsibilities.&nbsp;<\/p>\n\n\n\n<figure class=\"uwm-c-table\"><table Subfolders managed by OSEC, Graduate School and CIE.&nbsp;<\/td><td><\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-form-creation-nbsp\">Form Creation&nbsp;<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-use-system-prompts-nbsp\"><strong>Use System Prompts&nbsp;<\/strong><\/h3>\n\n\n\n<p>When creating or editing Slate forms,&nbsp;form owners&nbsp;must&nbsp;use system prompts for all standard fields.&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>What they do: System prompts&nbsp;contain&nbsp;default categories and values (e.g., program names, entry terms).&nbsp;<\/li>\n\n\n\n<li>Why they matter: Using system prompts ensures uniformity among forms and allows global updates\u2014when a value changes, all connected forms are updated automatically.&nbsp;<\/li>\n\n\n\n<li>Benefits:&nbsp;\n<ul class=\"wp-block-list\">\n<li>Reduces manual work&nbsp;when terms or programs change&nbsp;<\/li>\n\n\n\n<li>Improves data consistency across inquiries&nbsp;<\/li>\n\n\n\n<li>Ensures accuracy in reporting and list management&nbsp;<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-use-prompt-conditions-to-customize-nbsp\"><strong>Use Prompt Conditions to Customize&nbsp;<\/strong><\/h3>\n\n\n\n<p>Form managers should use&nbsp;prompt conditions to&nbsp;customize the values that show up in fields.&nbsp;For example, they may do this to have only certain majors or programs&nbsp;appear on a form.&nbsp;&nbsp;<\/p>\n\n\n\n<p>Form managers should use prompt conditions sparingly and only when there is a clear marketing need.&nbsp;When used, the&nbsp;form manager&nbsp;is responsible for&nbsp;reviewing the prompt conditions each semester and making sure they still serve their purpose.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-prompt-overrides-nbsp\"><strong>Prompt Overrides&nbsp;<\/strong><\/h3>\n\n\n\n<p>Prompt overrides are not&nbsp;permitted. Use prompt&nbsp;conditions instead&nbsp;to customize form fields.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-screening-for-international-inquiries-nbsp\"><strong>Screening for International Inquiries&nbsp;<\/strong><\/h3>\n\n\n\n<p>The Center for International Education has&nbsp;requested&nbsp;that all new&nbsp;RFI\/Web&nbsp;forms include this question: Do you need a student visa to study at 51ÁÔÆæ? Inclusion will allow 51ÁÔÆæ to&nbsp;identify&nbsp;students who should go into a comm flow for international students.&nbsp;<\/p>\n\n\n\n<p>Existing forms do not need to be retrofitted with this question, but if&nbsp;a&nbsp;form&nbsp;is being updated, please use the opportunity to add the question.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-form-fields-nbsp\"><strong>Form Fields&nbsp;<\/strong><\/h3>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-undergrad-nbsp-required-fields-nbsp\"><strong>Undergrad&nbsp;Required Fields&nbsp;<\/strong><\/h4>\n\n\n\n<p>These fields must be used on all undergraduate forms:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>First Name&nbsp;<\/li>\n\n\n\n<li>Last Name&nbsp;<\/li>\n\n\n\n<li>DOB&nbsp;<\/li>\n\n\n\n<li>Student Type&nbsp;<\/li>\n\n\n\n<li>Term&nbsp;<\/li>\n\n\n\n<li>Program&nbsp;<\/li>\n\n\n\n<li>Email&nbsp;<\/li>\n\n\n\n<li>Campus (hidden)&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-undergrad-nbsp-preferred-nbsp-fields-nbsp\"><strong>Undergrad&nbsp;Preferred&nbsp;Fields&nbsp;<\/strong><\/h4>\n\n\n\n<p>When possible, add these fields to undergraduate forms:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Phone (with phone comes the opt-in\/out question about text and AI outreach)&nbsp;<\/li>\n\n\n\n<li>Address&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-graduate-required-fields-nbsp\"><strong>Graduate Required Fields&nbsp;<\/strong><\/h4>\n\n\n\n<p>These fields must be used on all undergraduate forms:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>First Name&nbsp;<\/li>\n\n\n\n<li>Last Name&nbsp;<\/li>\n\n\n\n<li>Email&nbsp;<\/li>\n\n\n\n<li>Student Type&nbsp;<\/li>\n\n\n\n<li>Term&nbsp;<\/li>\n\n\n\n<li>Graduate Program of Interest&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-graduate-nbsp-preferred-fields-nbsp\"><strong>Graduate&nbsp;Preferred Fields&nbsp;<\/strong><\/h4>\n\n\n\n<p>When possible, add these fields to graduate forms:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Preferred Name&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-fields-that-should-not-be-used-nbsp-on-nbsp-graduate-forms\"><strong>Fields That Should Not Be Used&nbsp;on&nbsp;Graduate Forms<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>DOB&nbsp;<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-confirmation-emails\"><strong>Confirmation Emails<\/strong><\/h3>\n\n\n\n<p>All prospective students should receive some type of confirmation when they submit a form.&nbsp;Ideally, they would be redirected to a web page that confirms their submission <strong>and<\/strong> they would receive a confirmation email.&nbsp;<\/p>\n\n\n\n<p>Some programs use a Day 0 email instead of a confirmation email. This approach creates a risk: if a form is&nbsp;submitted&nbsp;during a specific window, no email is sent. Unless there is a clear need for a Day 0 email, programs should use a confirmation email.&nbsp;<\/p>\n\n\n\n<p>Confirmation&nbsp;emails should be separated for graduate and undergraduate audiences. Content should be broad enough to apply to students interested in any of the programs listed on the page.&nbsp;<\/p>\n\n\n\n<p>As a best practice, have graduate confirmation emails reviewed by the graduate recruitment lead in the Graduate School Office of Graduate Education (Elizabeth Farrow).&nbsp;<a href=\"https:\/\/admissions.uwm.edu\/portal\/OSECTICKET?tab=home\" target=\"_blank\" rel=\"noreferrer noopener\">Submit a ticket<\/a>&nbsp;to&nbsp;have&nbsp;undergraduate confirmation emails reviewed by the Office of Student Enrollment Communications.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-admin-notifications-nbsp\"><strong>Admin Notifications&nbsp;<\/strong><\/h3>\n\n\n\n<p>As a best practice, update the admin notify&nbsp;email&nbsp;field so form submissions trigger an alert for someone who can follow up personally. This may be a recruiter, program&nbsp;director&nbsp;or another staff member knowledgeable about the program and able to provide 1:1 outreach.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-ongoing-nbsp-form-nbsp-maintenance-nbsp\">Ongoing&nbsp;Form&nbsp;Maintenance&nbsp;<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-annual-form-audit-nbsp\"><strong>Annual Form Audit&nbsp;<\/strong><\/h3>\n\n\n\n<p>MarComm\u2019s&nbsp;Web team will conduct an annual audit of uwm.edu to create\/update an index of active Slate forms. It will document web page locations in the Slate notes field.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-annual-form-review\"><strong>Annual Form Review<\/strong><\/h3>\n\n\n\n<p>Managing units should review forms within their purview at least annually, retiring those no longer in use and updating any&nbsp;form&nbsp;fields with&nbsp;prompt&nbsp;conditions&nbsp;as needed.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-annual-confirmation-email-review\"><strong>Annual Confirmation Email Review<\/strong><\/h3>\n\n\n\n<p>Form&nbsp;managers&nbsp;should review confirmation emails&nbsp;at least&nbsp;annually and update them as needed. Rich content (photos, video) in confirmation emails&nbsp;should be no more than three years old.&nbsp;&nbsp;<\/p>\n\n\n\n<p>For photos, use the date the photo was&nbsp;take&nbsp;to&nbsp;determine&nbsp;its age.
