These instructions include the submission steps with screenshots of the form in I-Manager.<\/p>\n<p>You may also watch a <a href=\"https:\/\/cdnapisec.kaltura.com\/index.php\/extwidget\/preview\/partner_id\/2370711\/uiconf_id\/42910141\/entry_id\/1_q2wrn7s8\/embed\/dynamic\" target=\"_blank\" rel=\"noopener\">video demonstration<\/a> for starting and submitting a new IRB Application in I-Manager.<\/p>\n<p>To preview the questions, <a href=\"https:\/\/uwm.edu\/irb\/wp-content\/uploads\/sites\/127\/2021\/08\/New-IRB-Application-Form.pdf\" target=\"_blank\" rel=\"noopener\">click here to see a PDF of the IRB Application Form<\/a>. All potential questions are shown in the PDF. However, there is programming within the form, so many questions will appear only if you answer earlier questions a certain way when you\u2019re completing the form in I-Manager.<\/p>\n<p>Within the IRB Application Form, you will need to attach all relevant study materials, such as:<\/p>\n<ul>\n<li>Consent forms (<strong><a href=\"https:\/\/uwm.edu\/irb\/consent-form-templates\/\" target=\"_blank\" rel=\"noopener noreferrer\">Consent form templates<\/a><\/strong>)<\/li>\n<li>Recruitment materials<\/li>\n<li>Data collection instruments<\/li>\n<\/ul>\n<hr \/>\n<p>The IRB office has started to compile <a href=\"https:\/\/uwm.edu\/irb\/wp-content\/uploads\/sites\/127\/2021\/08\/New-IRB-Application-Form-FAQs.docx\">FAQs about the new IRB Application Form<\/a>.
