A copy of the ID will be retained along with the letter.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"a1\">Name on Your Paper Certificate<\/h2>\n\n\n\n<p>Your name will appear on your Paper Certificate as it appears on your <a href=\"https:\/\/uwm.edu\/graduateschool\/gradresources\/request-transcripts\/\" target=\"_blank\" rel=\"noreferrer noopener\">51ÁÔÆæ transcript<\/a>. To request changes to your name:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>You may submit a <a href=\"https:\/\/uwm.edu\/registrar\/forms\/diploma-name-request\/\">Diploma Name Request<\/a> to have a name printed on your Paper Certificate that is a slight variation of your legal name in PAWS (if you no longer have PAWS access, you will need to complete the <a href=\"https:\/\/uwm.edu\/onestop\/wp-content\/uploads\/sites\/82\/2019\/08\/Diploma-Name-Deviation-Request-PDF-1.pdf\">PDF version of the request<\/a>). No additional documentation will be required in these cases, but requests must be submitted prior to the commencement ceremony to allow time for review and processing. Paper Certificates are official documents, and this request is limited to minor changes or omissions such as:<ul><li>Full middle name to initial (ex. Andrew to A.).<\/li><li>Removal of middle name or middle initial (ex. Katie Marie Smith to Katie Smith).<\/li><li>Full first\/given name to shorter version (ex. Elizabeth to Beth).<\/li><li>Multiple first\/given or last\/family names to a single first and last name.<\/li><\/ul><strong>All Paper Certificate name requests are subject to review and approval by the Registrar\u2019s Office who reserves the right to decline any Paper Certificate name request without explanation.<\/strong><\/li>\n\n\n\n<li>If you want a significant change to your name, a different name, or a different spelling on your Paper Certificate, you will have to submit a <a href=\"https:\/\/uwm.edu\/finances\/wp-content\/uploads\/sites\/82\/2017\/04\/Student-Record-Data-Change-Form-Fillable.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Student Record Data Change Form<\/a> along with supporting documentation to the <a href=\"https:\/\/uwm.edu\/registrar\/contact-us\/\">Registrar\u2019s Office<\/a> at least 8 weeks before the commencement ceremony. Examples of situations that require documentation include:\n<ul class=\"wp-block-list\">\n<li>Middle initial to full name (ex. Jesse T.
