If a student requests to drop&nbsp;one or more courses while remaining enrolled in others, the medical circumstances must be especially well-documented to explain how their medical circumstances impact some courses but not all (e.g., a student breaks a leg and requests to drop a dance class because they cannot dance, a student cannot complete in-person courses while hospitalized but can complete their online courses, etc.).<\/p>\n\n\n\n<div class=\"uwm-u-flex uwm-u-flex--left\">\n<a class=\"uwm-c-button uwm-u-mr-20 uwm-u-mb-20\" href=\"https:\/\/cm.maxient.com\/reportingform.php?UnivofWisconsinMilwaukee&amp;layout_id=19\" rel=\"\">Medical Withdrawal and Tuition and Fee Appeal Request Form<\/a>\n<\/div>\n<\/div>\n\n\n\n<div class=\"uwm-l-col uwm-l-col--4 uwm-l-offset--1\"><div class=\"uwm-block-otp-nav \"><\/div><\/div>\n<\/div>\n<\/div><\/section>\n\n\n\n<section class=\"uwm-bg-gray uwm-u-py-80\"><div class=\"uwm-l-container--base uwm-l-container--overflow-hidden\">\n<h2 class=\"wp-block-heading heading-separator\" id=\"h-what-circumstances-qualify-for-a-medical-withdrawal\">What Circumstances Qualify for a Medical Withdrawal?<\/h2>\n\n\n\n<div class=\"uwm-p-card-grid uwm-l-grid uwm-l-grid--equal-y uwm-l-grid--2\">\n<div class=\"uwm-p-card uwm-p-card--gold uwm-u-mb-40\"><div class=\"uwm-p-card--body\"><div class=\"uwm-p-card--content\">\n<h3 class=\"wp-block-heading\" id=\"h-qualifying-circumstances\"><strong>Qualifying Circumstances<\/strong><\/h3>\n\n\n\n<p>A request may be granted to students who experience a serious and unexpected condition during the academic semester that completely prevents them from being able to function as a student. This includes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Experiencing a serious and unexpected physical or behavioral health condition.<\/li>\n\n\n\n<li>Needing to provide care to an immediate family member who is experiencing a serious or unexpected physical or behavioral health condition.<\/li>\n\n\n\n<li>Experiencing the death of an immediate family member.<\/li>\n<\/ul>\n\n\n\n<p>In the case of pre-existing, recurring, or chronic health conditions, documentation must show that the <strong>recurrence or worsening of the condition(s) began after initiation&nbsp;of the term for which the withdrawal is requested<\/strong>.<\/p>\n\n\n\n<p>Approval is not guaranteed, and will be granted on a case-by-case basis.<\/p>\n<\/div><\/div><\/div>\n\n\n\n<div class=\"uwm-p-card uwm-p-card--gold uwm-u-mb-40\"><div class=\"uwm-p-card--body\"><div class=\"uwm-p-card--content\">\n<h3 class=\"wp-block-heading\" id=\"h-non-qualifying-circumstances\"><strong>Non-Qualifying Circumstances<\/strong><\/h3>\n\n\n\n<p>Typically, requests are not granted to students who:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Did not participate in the course prior to the qualifying event.<\/li>\n\n\n\n<li>Are going through disciplinary proceedings or are suspended or expelled from 51ÁÔÆæ.<\/li>\n\n\n\n<li>Have experienced the death of a loved one who is not an immediate family member.<\/li>\n\n\n\n<li>Provide care to someone who is not an immediate family member.<\/li>\n\n\n\n<li>Experience medical situations that were known before the start of the semester with no substantial worsening in symptoms.<\/li>\n\n\n\n<li>Experience medical situations where the student could have reasonably anticipated or have taken more timely action.<\/li>\n\n\n\n<li>Experience medical situations for which the University could have provided accommodations.<\/li>\n<\/ul>\n<\/div><\/div><\/div>\n<\/div>\n<\/div><\/section>\n\n\n\n<section class=\"uwm-u-py-80\"><div class=\"uwm-l-container--base uwm-l-container--overflow-hidden\">\n<h2 class=\"wp-block-heading heading-separator\" id=\"h-is-a-medical-withdrawal-right-for-me\">Is a Medical Withdrawal Right for Me?<\/h2>\n\n\n\n<p>Sometimes dropping classes can have unanticipated consequences. Thus, a decision to drop classes or withdraw from the University should not be taken lightly. \u00a0We encourage students to first explore course accommodations, incomplete grades, or other options before dropping courses or withdrawing.\u00a0 When students are granted an \u201cincomplete\u201d from their instructor, they are given extra time (typically no longer than one semester) to complete the final assignments and receive a grade. See the\u00a0<a href=\"https:\/\/apps.uwm.edu\/secu-policies\/storage\/other\/SAAP%201-13.%20Incomplete%20Grades.pdf\">Incomplete Policy<\/a>\u00a0for more information.<\/p>\n\n\n\n<div class=\"uwm-p-card-grid uwm-l-grid uwm-l-grid--equal-y uwm-l-grid--2\">\n<div class=\"uwm-p-card uwm-p-card--gold uwm-u-mb-40\"><div class=\"uwm-p-card--body\"><div class=\"uwm-p-card--content\">\n<h3 class=\"wp-block-heading\" id=\"h-determine-if-a-medical-withdrawal-is-right-for-you\">Determine if a Medical Withdrawal is Right for You<\/h3>\n\n\n\n<p>To help, we encourage you to contact:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Your&nbsp;academic advisor&nbsp;or the&nbsp;<a href=\"https:\/\/uwm.edu\/academicaffairs\/facultystaff\/campus-contacts\/\">Assistant Dean or Director<\/a>&nbsp;in your school\/college to discuss dropping courses or withdrawing.<\/li>\n\n\n\n<li>A&nbsp;<a href=\"https:\/\/uwm.edu\/finances\/contact\/\">financial aid advisor<\/a>&nbsp;to discuss the implications that dropping classes or withdrawing may have on your financial aid award package.<\/li>\n\n\n\n<li>The&nbsp;<a href=\"mailto:dos@uwm.edu\">Dean of Students Office<\/a>&nbsp;to consult about Medical Withdrawals, to receive assistance in navigating this process, and to receive general support related to finances, health, community resources, and your overall success.<\/li>\n<\/ul>\n<\/div><\/div><\/div>\n\n\n\n<div class=\"uwm-p-card uwm-p-card--gold uwm-u-mb-40\"><div class=\"uwm-p-card--body\"><div class=\"uwm-p-card--content\">\n<h3 class=\"wp-block-heading\" id=\"h-things-to-consider-before-dropping-courses\">Things to Consider before Dropping Courses<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/uwm.edu\/onestop\/finances\/receiving-financial-aid\/\" target=\"_blank\" rel=\"noreferrer noopener\">Financial aid awards<\/a>. When students withdraw from or stop attending courses, their financial aid awards may be recalculated. If a student received a refund to cover living expenses, they may have to repay this. A tuition adjustment does not cover this balance.<\/li>\n\n\n\n<li>Future financial aid awards, especially with regard to completing, and meeting&nbsp;<a href=\"https:\/\/uwm.edu\/finances\/finances\/receiving-financial-aid\/satisfactory-academic-progress-sap\/#h-satisfactory-academic-progress-sap\">Satisfactory Academic Progress (SAP) requirements<\/a>.<\/li>\n\n\n\n<li>Repayment of outstanding student loans.<\/li>\n\n\n\n<li><a href=\"https:\/\/uwm.edu\/finances\/finances\/veterans-services\/\">Military benefits<\/a><\/li>\n\n\n\n<li>Assistantships, scholarships, grants, student-teaching placements, internships, etc.<\/li>\n\n\n\n<li>On-campus employment<\/li>\n\n\n\n<li>Access to&nbsp;<a href=\"https:\/\/uwm.edu\/housing\/\" target=\"_blank\" rel=\"noreferrer noopener\">on-campus housing<\/a>. Students must be enrolled in courses to live on-campus.<\/li>\n\n\n\n<li>Access to&nbsp;<a href=\"https:\/\/uwm.edu\/wellness\/\" target=\"_blank\" rel=\"noreferrer noopener\">Student Health &amp; Wellness Center<\/a><\/li>\n\n\n\n<li>Meeting academic degree time frame requirements<\/li>\n\n\n\n<li>Insurance premiums and eligibility<\/li>\n\n\n\n<li>Tax dependent status<\/li>\n\n\n\n<li>Visa status, study abroad, etc. Consult with the&nbsp;<a href=\"https:\/\/uwm.edu\/cie\/\" target=\"_blank\" rel=\"noreferrer noopener\">Center for International Education<\/a>&nbsp;if you are uncertain about how you may be impacted.<\/li>\n<\/ul>\n<\/div><\/div><\/div>\n<\/div>\n<\/div><\/section>\n\n\n\n<section class=\"uwm-bg-gray uwm-u-py-80\"><div class=\"uwm-l-container--base uwm-l-container--overflow-hidden\">\n<h2 class=\"wp-block-heading heading-separator\" id=\"h-what-is-the-deadline-to-apply-for-a-medical-withdrawal\">What is the Deadline to Apply for a Medical Withdrawal?<\/h2>\n\n\n\n<p>Students may apply for a&nbsp;<strong>retroactive medical withdrawal<\/strong>&nbsp;up to one year after the end of the term in question (e.g., requests for the spring 2023 must be submitted no later than the spring 2024 semester).<\/p>\n\n\n\n<p>Whenever possible, requests for medical withdrawals should occur during the term in which the medical condition arose. For applications submitted after the end of the term, transcript adjustments are the primary remedy; tuition adjustments may not be possible.<\/p>\n<\/div><\/section>\n\n\n\n<section class=\"uwm-u-py-80\"><div class=\"uwm-l-container--base uwm-l-container--overflow-hidden\">\n<h2 class=\"wp-block-heading heading-separator\" id=\"h-how-do-i-apply-for-a-medical-withdrawal\">How do I Apply for a Medical Withdrawal?<\/h2>\n\n\n\n<p><strong>Complete the&nbsp;<a href=\"https:\/\/cm.maxient.com\/reportingform.php?UnivofWisconsinMilwaukee&amp;layout_id=19\" rel=\"noreferrer noopener\" target=\"_blank\">Medical Withdrawal Application<\/a>.<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Drop course(s)<\/strong>. If you are certain about your decision to drop your courses and it is before the&nbsp;<strong><a href=\"https:\/\/uwm.edu\/registrar\/dates-deadlines\/add-drop-calendar\/\">drop deadline<\/a><\/strong>, drop your courses in PAWS before submitting the application.<\/li>\n\n\n\n<li><strong>Provide a statement.<\/strong>&nbsp;Within your application, provide a statement regarding the circumstances that necessitate your withdrawal. If you are requesting a drop for a few courses (rather than all courses for the semester) explain the specific reason(s) why your situation or medical circumstances only impacted some courses but not all (e.g., broke bones and cannot complete dance class; cannot complete in-person courses while hospitalized but can complete online courses, etc.).<\/li>\n\n\n\n<li><strong>Attach Supporting Documentation.&nbsp;<\/strong>Documentation must be from a licensed healthcare provider or professional victim advocate.&nbsp;See below.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-documentation\">Documentation<\/h3>\n\n\n\n<p>In the case of death of an immediate family member, an&nbsp;<strong>obituary or death certificate<\/strong>&nbsp;may be submitted in lieu of a statement from a licensed healthcare provider.<\/p>\n\n\n\n<p>For all other circumstances, documentation must be from a licensed healthcare provider or professional victim advocate. Documentation must:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Be on&nbsp;<strong>official letterhead<\/strong>&nbsp;or include the healthcare provider\u2019s&nbsp;<strong>contact information<\/strong>.<\/li>\n\n\n\n<li>Include<strong>&nbsp;date(s)<\/strong>&nbsp;when you experienced the serious and unexpected condition and when you saw your provider. Dates must pertain to the semester of the appeal.<\/li>\n\n\n\n<li>Include a&nbsp;<strong>statement<\/strong>&nbsp;that you experienced a&nbsp;<strong>serious or unexpected condition<\/strong>&nbsp;during the academic semester that completely&nbsp;<strong>prevents&nbsp;you from being able to function as a student<\/strong>. This must include that you:\n<ul class=\"wp-block-list\">\n<li>experienced a serious or unexpected physical or behavioral health condition,<\/li>\n\n\n\n<li>need to provide care to an&nbsp;immediate family member&nbsp;who is experiencing a serious or unexpected physical or behavioral health condition, or<\/li>\n\n\n\n<li>experienced the death of an&nbsp;immediate family member.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>In the case of pre-existing, recurrence, or worsening of condition(s), documentation must show that the <strong>recurrence or worsening began after the start of the term<\/strong>&nbsp;for which the withdrawal is requested.<\/li>\n\n\n\n<li>In cases where&nbsp;<strong>dropping one or a few courses<\/strong>&nbsp;(rather than all courses for the semester) is requested, documentation should explain the selective nature of the academic impact.<\/li>\n<\/ul>\n\n\n\n<p>If your circumstances don\u2019t quite fit within the above criteria, please have your healthcare provider do their best in explaining why they think you would qualify.<\/p>\n\n\n\n<p>All documentation must be submitted within&nbsp;<strong>one year from the semester of withdrawal\/drop for consideration<\/strong>. We consider all requests that are submitted by the appeal deadline.&nbsp;&nbsp;Documentation can also be emailed to&nbsp;<a href=\"mailto:dos@uwm.edu\">dos@uwm.edu<\/a>.<\/p>\n<\/div><\/section>\n\n\n\n<section class=\"uwm-bg-gray uwm-u-py-80\"><div class=\"uwm-l-container--base uwm-l-container--overflow-hidden\">\n<h2 class=\"wp-block-heading heading-separator\" id=\"h-what-happens-after-i-request-a-medical-withdrawal\">What Happens After I Request a Medical Withdrawal?<\/h2>\n\n\n\n<p>Medical Withdrawal requests undergo a multi-step review. Applicants will receive updates regarding which step in the review process their application is in.&nbsp;&nbsp;Students who have already dropped their courses in PAWS will skip Step 1 and go straight to Step 2.<\/p>\n\n\n\n<div class=\"uwm-p-card-grid uwm-l-grid uwm-l-grid--equal-y uwm-l-grid--3\">\n<div class=\"uwm-p-card uwm-u-mb-40 uwm-p-card--gold\"><div class=\"uwm-p-card--body\"><div class=\"uwm-p-card--title\">Step 1<\/div><div class=\"uwm-p-card--content\">The Medical Withdrawal Committee reviews the application to decide whether a Medical Withdrawal shall be granted.\u00a0Students may be contacted via email and asked to provide additional information for review.<\/div><\/div><\/div>\n\n\n\n<div class=\"uwm-p-card uwm-u-mb-40 uwm-p-card--gold\"><div class=\"uwm-p-card--body\"><div class=\"uwm-p-card--title\">Step 2<\/div><div class=\"uwm-p-card--content\">In cases where a Medical Withdrawal is approved by the Medical Withdrawal Committee and in cases when students drop courses on their own in PAWS, the Dean of Students Office reviews the application to determine proration of tuition and segregated fees. For applications submitted after the end of the term, tuition adjustments may not be possible.<br><br>Students may be contacted via email and asked to provide additional information for review or to meet with a Dean of Students Office Coordinator.<br><\/div><\/div><\/div>\n\n\n\n<div class=\"uwm-p-card uwm-u-mb-40 uwm-p-card--gold\"><div class=\"uwm-p-card--body\"><div class=\"uwm-p-card--title\">Step 3<\/div><div class=\"uwm-p-card--content\">In cases where tuition and fees are prorated, the Student Financial Accounts Office will make adjustments to the student\u2019s PAWS account. This step typically takes about 3 weeks to be completed.<\/div><\/div><\/div>\n<\/div>\n<\/div><\/section>\n\n\n\n<section class=\"uwm-u-py-80\"><div class=\"uwm-l-container--base uwm-l-container--overflow-hidden\">\n<h2 class=\"wp-block-heading heading-separator\" id=\"h-what-happens-to-my-transcripts-if-i-am-granted-a-medical-withdrawal\">What Happens to my Transcripts if I am Granted a Medical Withdrawal?<\/h2>\n\n\n\n<p>The last date of class attendance shall be considered the official withdrawal date to be used by the Registrar\u2019s Office for recording academic record class drops and term withdrawals. Based on the date, dropped classes will either be removed entirely from your transcripts, or if your last date of attendance was after the W deadline (25% or more through the course), the courses will be listed on your transcript with a \u201cW\u201d notation. The \u201cW\u201d notation stands for withdrawal and it serves a record keeping function. \u201cW\u201d notations have no impact on your GPA or ability to re-enroll in classes in a future term. This date will also be used by the Financial Aid Office to make award and\/or repayment decisions,&nbsp;<strong>which could result in a change to the student\u2019s overall financial aid<\/strong>.<\/p>\n<\/div><\/section>\n\n\n\n<section class=\"uwm-bg-gray uwm-u-py-80\"><div class=\"uwm-l-container--base uwm-l-container--overflow-hidden\">\n<h2 class=\"wp-block-heading heading-separator\" id=\"h-how-are-tuition-and-fees-prorated-reduced-if-i-am-granted-a-medical-withdrawal\">How are Tuition and Fees Prorated (reduced) if I am Granted a Medical Withdrawal?<\/h2>\n\n\n\n<p>All students are expected to thoroughly read the 51ÁÔÆæ financial policies including reviewing the&nbsp;<a href=\"https:\/\/uwm.edu\/onestop\/dates-and-deadlines\/interactive-adddrop-calendar\/\" rel=\"noreferrer noopener\" target=\"_blank\">Interactive Add\/Drop Calendar<\/a>&nbsp;for information on the&nbsp;<a href=\"https:\/\/uwm.edu\/registrar\/dates-deadlines\/add-drop-calendar\/\" rel=\"noreferrer noopener\" target=\"_blank\">fee assessments<\/a>&nbsp;for dropping or withdrawing from classes.&nbsp;All students, regardless of circumstances, are subject to the published 51ÁÔÆæ tuition and fee policies.&nbsp;These policies are guided by the&nbsp;<a href=\"https:\/\/www.wisconsin.edu\/uw-policies\/uw-system-administrative-policies\/tuition-and-fee-policies-for-credit-instruction\/\" rel=\"noreferrer noopener\" target=\"_blank\">UW System Tuition and Fee Policies<\/a>.&nbsp;Refund policies related to dropping or withdrawing from courses are found under&nbsp;<a href=\"https:\/\/www.wisconsin.edu\/uw-policies\/uw-system-administrative-policies\/tuition-and-fee-policies-for-credit-instruction\/#C4\" rel=\"noreferrer noopener\" target=\"_blank\">C4: Refunds\/Withdrawals\/Drops<\/a>.&nbsp;<\/p>\n\n\n\n<p>In some instances, a Medical Withdrawal may also result in the proration of tuition and segregated fees for that term. This process is in accordance with the&nbsp;<a href=\"https:\/\/www.wisconsin.edu\/uw-policies\/uw-system-administrative-policies\/medical-withdrawal-policy\/\" target=\"_blank\" rel=\"noreferrer noopener\">UW System Medical Withdrawal Policy, 807<\/a>. <\/p>\n\n\n\n<p>In cases where a proration of tuition and fees is in order, the following tables will be used:<\/p>\n\n\n\n<div class=\"uwm-l-row\">\n<div class=\"uwm-l-col\">\n<h3 class=\"wp-block-heading\" id=\"h-fall-amp-spring-full-semesters\">Fall &amp; Spring Full Semesters<\/h3>\n\n\n\n<figure class=\"uwm-c-table uwm-c-table--stripe\"><table class=\"has-fixed-layout\"><thead><tr><th>Weeks into Session<\/th><th>Refund Range<\/th><\/tr><\/thead><tbody><tr><td>0-2 weeks<\/td><td>100%<\/td><\/tr><tr><td>3-4 weeks<\/td><td>70-90%<\/td><\/tr><tr><td>5-8 weeks<\/td><td>50-70%<\/td><\/tr><tr><td>9-12 weeks<\/td><td>30-50%<\/td><\/tr><tr><td>13 weeks or more<\/td><td>10-30%<\/td><\/tr><\/tbody><\/table><\/figure>\n<\/div>\n\n\n\n<div class=\"uwm-l-col\">\n<h3 class=\"wp-block-heading\" id=\"h-winter-term-summer-and-other-non-standard-length-courses\">Winter Term, Summer, and other Non-Standard-Length Courses<\/h3>\n\n\n\n<figure class=\"uwm-c-table uwm-c-table--stripe\"><table class=\"has-fixed-layout\"><thead><tr><th>Weeks into Session<\/th><th>Refund Range<\/th><\/tr><\/thead><tbody><tr><td>First week<\/td><td>100%<\/td><\/tr><tr><td>Second week<\/td><td>70-90%<\/td><\/tr><tr><td>3-4 weeks<\/td><td>50-70%<\/td><\/tr><\/tbody><\/table><\/figure>\n<\/div>\n<\/div>\n\n\n\n<p>In instances where tuition and fees for the term are paid, refunds are returned to the original source of payment. If tuition and fees were paid via loans, scholarships, grants, or a third party, any refund as a result of the proration of tuition and fees may be returned to the lender, scholarship provider, grant funder, or third party respectively.<\/p>\n\n\n\n<p>Please note that tuition and fee adjustments only apply to balances related to tuition and fees for the semester of the request. Adjustments associated with a Medical Withdrawal do not apply to University Housing and Meal Plan changes, library fees, fines, recalculation of financial aid, or other balances. Refund or proration of other fees is subject to existing university policy.<\/p>\n<\/div><\/section>\n\n\n\n<section class=\"uwm-u-py-80\"><div class=\"uwm-l-container--base uwm-l-container--overflow-hidden\">\n<h2 class=\"wp-block-heading heading-separator\" id=\"h-what-happens-to-my-financial-aid-if-i-am-granted-a-medical-withdrawal\">What Happens to my Financial Aid if I am Granted a Medical Withdrawal?<\/h2>\n\n\n\n<p>The last date of class attendance shall also be used by the Financial Aid Office to make award and\/or repayment decisions. Please note that if you are covering all or part of your charges with financial aid and you withdraw from all of your courses,&nbsp;<strong>you may be required to return a portion of your award<\/strong>.<\/p>\n\n\n\n<p>Financial Aid is awarded to students under the assumption that they will complete the courses they are enrolled in.&nbsp; This means that each day a student is enrolled and attending classes, they \u201cearn\u201d a portion of their financial aid.&nbsp; When a student withdraws from all classes before completing at least 60% of the semester, they have not earned all of their financial aid, and a portion may be sent back to the Department of Education. If you are certain about your decision to drop your courses and it is before the&nbsp;<strong><a href=\"https:\/\/uwm.edu\/registrar\/dates-deadlines\/add-drop-calendar\/\">drop deadline<\/a><\/strong>, drop your courses in PAWS before submitting the application.<\/li>\n\n\n\n<li><strong>Provide a statement.<\/strong>&nbsp;Within your application, provide a statement regarding the circumstances that necessitate your withdrawal. If you are requesting a drop for a few courses (rather than all courses for the semester) explain the specific reason(s) why your situation or medical circumstances only impacted some courses (e.g., broke bones and cannot complete dance class; cannot complete in-person courses while hospitalized but can complete online courses, etc.).<\/li>\n\n\n\n<li><strong>Attach Supporting Documentation.&nbsp;<\/strong>Documentation must be from a licensed healthcare provider or professional victim advocate:<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-documentation-0\">Documentation<\/h3>\n\n\n\n<p>In the case of death of an immediate family member, an&nbsp;<strong>obituary or death certificate<\/strong>&nbsp;may be submitted in lieu of a statement from a licensed healthcare provider.<\/p>\n\n\n\n<p>For all other circumstances, documentation must be from a licensed healthcare provider or professional victim advocate. Documentation must:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Be on&nbsp;<strong>official letterhead<\/strong>&nbsp;or include the healthcare provider\u2019s&nbsp;<strong>contact information<\/strong>.<\/li>\n\n\n\n<li>Include<strong>&nbsp;date(s)<\/strong>&nbsp;when you experienced the serious and unexpected condition and when you saw your provider. Dates must pertain to the semester of the appeal.<\/li>\n\n\n\n<li>Include a&nbsp;<strong>statement<\/strong>&nbsp;that you experienced a&nbsp;<strong>serious or unexpected condition<\/strong>&nbsp;during the academic semester that completely&nbsp;<strong>prevents&nbsp;you from being able to function as a student<\/strong>.
