Access to this system cannot be granted until after initial documentation is received and reviewed, and students will need to complete these modules prior to the start of rotations.\u00a0 More information with instructions for students will be sent after all of the documents below have been submitted to the clinical site for review.<\/p>\n<hr \/>\n<h1 style=\"text-align: center\">Documents to complete<\/h1>\n<p style=\"text-align: center\">Please review and complete the following forms and forward to Brad De Pons (bkdepons@uwm.edu).\u00a0 Materials MUST be received by Brad in order to submit to your clinical site <strong>NO LATER THAN SEPTEMBER 13th, 2024<\/strong> (6 weeks prior to the start of clinical rotations).\u00a0 Forms may be submitted to Brad via email at bkdepons@uwm.edu, or paper copies can be dropped off.\u00a0 <strong>Paper copies are preferred to ensure high quality scans.<\/strong>\u00a0 If submitting via email, please submit good quality scans of each document.<\/p>\n<hr \/>\n<p><a href=\"https:\/\/uwm.edu\/bms-labs\/wp-content\/uploads\/sites\/361\/2023\/07\/8.-Non-AAH-Employees-Confidentiality-Agreement-ACL.pdf\">Non-AAH Employees Confidentiality Agreement-ACL<\/a> &#8211;
