If the incident involves any type of personal injury or property damage:<\/strong><\/span><\/h4>\n

STUDENTS \/ VOLUNTEERS \/ VISITERS \/ GUESTS:<\/strong> Complete the university General Incident Report<\/a> along with the BMS Accident Incident Report Form<\/a>.<\/p>\n

EMPLOYEES:<\/strong> Complete the university Employer’s First Report of Injury or Disease<\/a> form along with the BMS Accident Incident Report Form<\/a>.<\/p>\n

*Supervisor(s) must complete and submit all campus reporting forms within 24 hours.<\/p>\n

<\/h5>\n

If the incident does NOT<\/strong> involve personal injury or property damage, but could have<\/em> resulted in injury or damage:<\/span><\/h4>\n

Complete the\u00a0BMS Accident Incident Report Form<\/a> only.<\/p>\n

 <\/p>\n

A.<\/strong> Information collected for reports must include:<\/p>\n

a.<\/strong> Name\/contact information of exposed individual
\nb.<\/strong> Date\/time\/location of incident
\nc.<\/strong> Description of incident, along with detail of injuries sustained
\nd.<\/strong> Type of exposure (blood, body fluid, needle containing xxx, etc.), including identification of source if available
\n.\u00a0<\/span>\u00a0\u00a0\u00a0 i.<\/strong> If possible, original sample should be retained if needed for future testing\/further assessment
\ne.<\/strong> Name\/contact information of any witnesses to incident
\nf.<\/strong> Description of any safety violations or variation in protocol that may have led to incident
\ng.<\/strong> Treatment rendered, i.e. flushed wound, used eyewash station, etc.<\/p>\n

B.<\/strong> Worker’s Compensation policy and reporting forms for employees can be found\u00a0on the 51ÁÔÆæ Human Resources<\/a> website, and can also be found in section VIII part C of the Chemical Hygiene Plan [SAFE 7.0]<\/a>.<\/p>\n

C. <\/strong>See Exposure Control Plan [SAFE 6.0]<\/a> for instructions if the incident involves an exposure to blood, body fluids, or other potentially infections materials.\u00a0<\/strong><\/p>\n

D.<\/strong> It is recommended that all forms are completed immediately. If unable to complete at time of exposure, forms must be completed and submitted within 24 hours of incident.<\/p>\n

a.<\/strong> General Incident Report<\/a><\/span> must be submitted to the UW-Milwaukee, Department of University Safety & Assurances (address located on form).<\/p>\n

b.<\/strong> Employer’s First Report of Injury or Disease<\/a><\/span> must be submitted to the Department of Workforce Development Worker\u2019s Compensation Division (address located on form).<\/p>\n

c.<\/strong> BMS Accident Incident Report Form<\/span><\/a> is automatically sent to the Laboratory Manager, and will be forwarded to the Department of Biomedical Sciences Program Director and placed in the student file (if applicable).<\/p>\n
